Patient A:

Chief Complaint: H/o of course of antibiotics, IBS, irregular periods, weight gain

Stats: 32 yo, Female, 5’5, 180 pounds

Summary: Patient reported difficulty losing weight and managing digestive symptoms. Was following a low calorie diet and exercising 5 x per week.

Conclusion/Treatment Plan: Determined that long courses of antibiotics upset the bacteria in the gut causing persistent gas, bloating. Patient was also having difficulty breaking down foods and upon using digestive enzyme support and specific probiotics, digestive symptoms were relieved. An anti yeast diet was prescribed as well as natural anti fungals and after 3 months hormones were rebalanced. Patient was also overexercising and upon adjusting activity to less frequent duration and more challenging intervals, she began to lose weight.

Chief Complaint: H/o anxiety, sleep irregularities, weight gain, mood swings, poor focus/concentration

Stats: 48 yo, Female

Conclusion/Treatment Plan: determined that chronic sleep problems were due to elevated cortisol levels at night;
